Job Description

Provides daily supportive and functional care to residents in a compassionate environment according to established community care procedures as directed by nursing leadership. Works as a collaborative member of an interdisciplinary team that focuses on the resident as the director of their own care. Responsibilities include maintaining residents’ comfort, providing physical care with activities of daily living, answering call lights and requests, reporting observations to the nurse, handling personal care needs, observing residents for changing needs, participating in care/service plans, assisting during mealtimes, creating meaningful relationships with residents and families, supporting residents’ decision making, assisting with therapies and treatments, documenting care, and maintaining a clean and safe environment. Requires current CNA certification (Missouri, Illinois, or Pennsylvania with specific conditions). Physical demands include standing, walking, lifting up to 50 lbs, and working in a well-lighted, ventilated area with exposure to various conditions and substances. The job may require working beyond normal hours, weekends, and holidays. The description notes that other duties may be assigned as needed.
