SUMMARY

Skills Trainers will provide skills training to program youth directly related to the goals of the youth's treatment plan; Skills Trainers will act as the youth's advocate, mentor, and behavior coach, and attend wrap meetings for coordination of care. The average caseload is 10- 12 youth. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. ESSENTIAL DUTIES AND RESPONSIBILITIES (Other duties may be assigned)
  • Provide rehabilitation services to youth in the home, school, and community focusing on specific skill deficit areas including, independent living skills, emotion regulation, social skills, and coping skills
  • Helps develop formal/informal support networks for youth/families in the communities in which they reside
  • Participates in wraparound team meetings as needed
  • Acts as a positive role model for youth and promotes greater youth participation and voice at the practice, program, and policy levels of the System of Care
  • Documents each skills training session per Medi-Cal requirements and SDCC policies and procedures
  • Maintains clear, effective, and open communication between multi-disciplinary clinical staff and administrative personnel within and between SDCC programs
  • Utilizes supervision, consultation, and training as necessary to provide high-quality services to youth in an effective manner
  • Availability to work evening and occasional weekend hours, and to be on-call during working hours by the youth on their caseload
  • Fulfills all required training including CMHS Family Partnership Training, applicable EBP training, cultural competency, and HIPAA

Ability to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groups

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.

MINIMUM REQUIREMENTS:
  • Valid California Driver's License and reliable transportation
  • Bachelor's Degree from an accredited University AND/OR experience as a consumer of Children's Behavioral Health Services, Child Welfare Services, the Juvenile Justice System, or Special Education
  • Good verbal and written communication skills, and the ability to work effectively within a multi-disciplinary team
  • Must be 21 years of age or older.
PREFERRED REQUIREMENTS:
  • Proficient in SmartCare (online medical records system)
  • Experience as a youth advocate, educator, or mental health counselor
  • Bilingual in Spanish
  • Experience working in a wraparound program or with foster/probation youth
  • Certified peer support specialist certification
