LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Washington, District of Columbia.

Job Description & Requirements

#HCRR (PP).

Requirements:
- 2 years of experience required: Acute Hospital, Long Term Acute Care/Rehab/Skilled Nursing, Admission Criteria, experience in Care coordination, Discharge Planning, Utilizing InterQual Criteria, and Utilizing Milliman Guidelines; previous Charge experience preferred
- Regulatory skills required: knowledge of CPT codes, Department of Health DRG, HEDIS measures, NCQA, OSHA procedures and JCC
- Active RN License to work in DC 
-BLS

DC District requirement: MUST be able to WORK in office at Tilden Location
Tilden Office Location: Connecticut street and Tilden Street in NW DC

*This is an in-office setting in Washington DC. Some face to face will be required at the wellness center off Minnesota avenue in SE DC. Training and office work are located off Connecticut street and Tilden Street in NW DC*

Unit Information:
- Chronic diagnoses (High Risk Pregnancy, Postpartum, DM, CHF, COPD, Asthma, obesity/overweight, All Behavorial Health, etc.)
- CM make calls to enrollees in Washington DC area to complete comprehensive assessments, developing care plans, and ongoing management within the EMR (guiding care/Health edge).
- Dress Code: business casual
